1. Acceptance of Terms

Welcome to Mayflower Behavioral Health LLC ("Practice", "we", "us", or "our"), founded and clinically directed by Azeezat A. Samuel, MSN, PMHNP-BC. These Terms of Service ("Terms") govern your use of our website (mayflowerbehavioralhealth.com), our integrated scheduling portals, and your participation in outpatient psychiatric clinical services.

By accessing this website or scheduling an appointment, you acknowledge that you have read, understood, and agreed to be bound by these Terms and our Privacy Policy & Notice of Privacy Practices. If you do not agree to these Terms, please do not use our website or services.

2. Telehealth Informed Consent

Mayflower Behavioral Health delivers psychiatric evaluations, ongoing psychopharmacological medication management, and supportive psychotherapy via synchronous, secure audiovisual telehealth technology.

By engaging in telehealth services with our practice, you acknowledge and agree that:

• Nature of Telehealth: Telehealth involves electronic transmission of video, audio, and clinical data across secure interactive interfaces. You will have direct face-to-face communication with your clinician.
• Benefits & Limitations: Benefits include convenient, timely access to psychiatric care from your home or private office. Potential limitations include technical connectivity failures and the inability to perform physical in-person examinations.
• Alternative Options: You have the right to seek in-person psychiatric care elsewhere at any time.
• Emergency Preparedness: At the start of every telehealth visit, you must verify your exact physical address and provide a reliable emergency contact phone number. If a safety crisis arises during a session, our provider may contact local emergency first responders in your area.

4. Appointments, Financial Agreement & Cancellation Policy

To maintain clinical availability for all patients requiring psychiatric attention, Mayflower Behavioral Health enforces the following scheduling and financial guidelines:

• Payment Due at Service: Copayments, coinsurance, deductibles, or private-pay visit fees are due at the time the appointment is conducted. Valid credit card information must be securely kept on file via our encrypted payment processor.
• 24-Hour Cancellation Policy: We require at least 24 hours advance notice to cancel or reschedule an appointment without penalty.
• Cancellation & No-Show Fees: Appointments cancelled with less than 24 hours notice or missed entirely ("no-shows") are subject to an administrative fee:
  • $100 Fee: Established patient follow-up visit.
  • $150 Fee: New patient initial psychiatric evaluation.
  Note: Commercial health insurance and Medicaid/Medicare do not reimburse cancellation or no-show fees. The patient is solely responsible for payment.
• Late Arrivals: Arriving greater than 10 minutes past your scheduled appointment time may necessitate rescheduling, and late cancellation fees may apply.

5. Prescriptions & Controlled Substances Policy

Psychiatric medication prescribing is guided strictly by evidence-based medicine, patient safety, and statutory drug enforcement regulations:

• No Guaranteed Prescriptions: An evaluation does not guarantee that a specific medication or controlled substance will be prescribed. Prescriptions are provided solely when clinically indicated following comprehensive assessment.
• Controlled Substances (DEA Schedules II–V): Prescriptions for controlled substances (e.g., stimulant ADHD medications, benzodiazepines, sedative-hypnotics) are subject to rigorous federal DEA, Ryan Haight Act, and state Prescription Drug Monitoring Program (PDMP) oversight. Regular monitoring appointments and random urine toxicology screens may be mandated.
• Refill Requests: Routine medication refills require a minimum of 48 to 72 business hours advance notice. Refills will not be processed outside of normal business hours or on weekends.
• Lost or Stolen Medications: Early refills for lost, damaged, or stolen controlled substances are not provided under any circumstances.

The materials, blog articles, FAQs, and service descriptions presented on this website are provided solely for educational and informational purposes. They do not constitute formal psychiatric advice, medical diagnosis, or establish a provider-patient relationship.

A formal provider-patient relationship is established only after completion of an initial clinical intake, mutual execution of practice consent documents, and direct clinical evaluation by Azeezat A. Samuel, PMHNP-BC.

All content, text, trademarks, brand logos, graphic design, and custom styling on this website are the proprietary property of Mayflower Behavioral Health LLC and are protected by United States copyright, trademark, and unfair competition laws.

You may not reproduce, republish, distribute, reverse-engineer, or scrape any portion of this website without our express written permission.

8. Limitation of Liability

To the fullest extent permitted by applicable law, Mayflower Behavioral Health LLC, its officers, clinicians, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of your access to or inability to access this website, technical telecommunication interruptions, or unauthorized third-party access to digital transmissions.

9. Governing Law, Dispute Resolution & Contact

These Terms shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to its conflict of law principles. Any legal action or proceeding arising under these Terms shall be instituted exclusively in the state or federal courts located in the State of Georgia.
